A model of a referral form for assistance from a UK food bank .Food banks are a relatively recent phenomenon in the UK, necessitated by increasing umbers of people who have insufficient money to buy food. This often occurs when people claiming state benefits are "sanctioned" ie have benefit suspended for a period, or are deemed ineligible or due to delays in processing a claim. Generally, food bank organisers require people to be referred by organisations rather than arriving and asking for food themselves, hence the existence of the forms though some do allow self-referrals. This is a generic model of one, somewhat simplified, not an actual form, but it is based on real one.
