The TRUSS-FITTING. 171 advantages of the English truss are well shown in fig. 84, asapplied to double inguinal hernia; we believe, however, thatwith accurate fitting of the spring, the understraps, which areahvajs objectionable, could usually be abandoned, and we canspeak in praise of the position of the pads only. In contrastto this, attention is called to a truss of the same type (fig. 85), Fig. An English type of double truss, applied. {Eccles.) very extensively sold in this country, which, while excellent inconstruction, has the serious defect of ha\-ing the centre of thepad too far below the spring. This truss, as shown applied, isin very good position, except that the spring is too the wearer stoops, the abdomen strikes the upper edge ofthe spring, and forces the pad down over the pubic bone. 172 ABDOMINAL HERXL\. Direct Inguinal Hernia.—The mechanical treatment ofdirect hernia is, in most respects, similar to that of the obliquevariety, except that the difficulties are somewhat increased onaccount of its close proximity to tlie pubic bone. Tlie con-tents of a direct iiernia may sometimes account for truss-wear-ing being more painful. In direct hernia we may have thebladder, caecum, or sigmoid flexure, forming part or all of theprotrusion. In such a case pain is caused by direct truss-pressure upon the l:)ladder or bowel.
