The extension may be performed in the same direction with theaid of the pulleys;—recollecting always that they are not to be used inorder to exert greater force, but to exert it more equably. A damp ban-dage should be applied round the elbow to protect the skin before thestrap of the pulleys is attached. Fig. (3.) By the heel in the axilla. The patient lies down on a bed, andthe surgeon sits on the edge. He puts his heel (without his boot) intothe axilla, to press the head of the bone upwards and outwards, and atthe same time pulls the limb downwards by means of a towel fastenedround the elbow.
