. Journal of radiology . r the sudden change wasnoticed. Necropsy by Dr. W. C. Caldwell—His notes are as follows: The Brewertube and the drains were removed. Onintroducing the hand into the cavitythrough the incision, it was found thatthe large cyst had occupied practicallythe entire space of the right lung, theright lung being pushed up into a smallspace in the region of the clavicle andapex. The cyst extended from themediastinum to the liver, to the chestwall, and almost to the clavicle. Therewas a definite pneumothorax as thecyst had decreased somewhat in size, since drainage. On attempting to de-tach the cyst, it was found to be veryadherent above, in the region of theclavicle, but particularly in the medias-tinum, where it was so firmly attachedthat one was unable to separate it withthe gloved hand. On opening the cystit was found to contain a tumor massabout the size of an orange attachedat the upper portion of the cyst. Thistumor mass was removed. • It was im-possible to remove the cyst wall intact,. Figure XII.—Three weeks later, spontaneous rupture into bronchuswith disappearance of shadow. 95 X-RAY STUDIES OF MtiDIASTXAL SHADOWS IN DERMOID CYST—KAHN but a portion of the cyst wall was re-moved for examination. Examinationoi the small compressed lung showedwhat was apparently red block of this was removed for ex-amination. There was now a cavity inthe position formerly occupied by theright lung as large as a childs head. Cross Pathology—The tumor masswhich was irregular in outline, was softand spongy in consistency, had a pink-ish white appearance and looked verymuch like skin. It was covered withshort fine hair. On section, the skinsurface had a thin layer of connectivetissue beneath, very similar to normalskin. Deeper down there was consider-able fat and a connective tissue of the cyst wall showed a verydense hard fibrous tissue almost likecartilage. Piece of lung showed con-solidation.
