Attacks of acute rheumatism are brought on inmost instances by exposure to cold, by getting wet through, some-times also by excessive fatigue, such as walking long distances,especially if in addition there have been overheating of the bodyand subsequent chill. Persons who have once suffered from thisdisease are very liable to a recurrence on a renewal of the excit-ing cause, and, even apparently independently of this, from suchcauses as digestive disturbances. Symptoms.—An attack of acute rheumatism is usually ush-ered in by chilliness or rigors, followed with feverishness, and a ACUTE RHEUMATISM 1281. feeling of stiffness or pain in one or more joints, generally thoseof larger or medium size, such as the knees, andsometimes in the lower limbs, as shown in theaccompanying figure. It soon becomes intense,and is accompanied by severe constitutional dis-turbance and prostration. The patient lies help-less in bed, restless, but afraid to move or to betouched, and unable to bear even the weight ofthe bedclothes. The face is flushed, and thewhole body bathed in perspiration, which has ahighly acid reaction and a sour, disagreeableodor. The temperature is markedly elevated(103° to 105°), the pulse rapid, full, and soft; the tongue iscoated with a yellow fur, and there are thirst, loss of appetite,and constipation.
