Another class of cases is to be met with in which the vagina islargely occupied by a soft, friable, and bleeding cancerous mass, or anenormously large and indurated cervix (Figs. 22 and 23). In such con-dition it will be found impossible to form a correct idea as to the extentto which the diseas,e may have invaded the uterus until the obstructingmass has been first removed by the cautery loop, the sharp curette, orboth. This having been effected, the upper vaginal walls previouslyhidden should be carefully examined and any suspicious-looking nodules. Should itbe found that the disease in the posterior lip has involved the cul-de-sacof Douglas to a degree sufficient to warrant its excision and conse-quently opening into the peritoneal cavity, this had better be made thefinal proceeding in the operation. If carefully done, no evil consequenceswhatever may be anticipated therefrom, an antiseptic tampon to pre-vent intestinal protrusion being sufficient to meet the emergenc}^ (Fig-24). The depth of the uterus should now be taken, what remains of thecanal forcibly dilated and scraped out bj^ sharp curette, and the uterinecavit}^ if necessary, submitted to the same process.
