An introduction to the practice of midwifery. ear or backpart of the neck cannot be felt, the os externum must be gra-dually dilated in the time of the pains, with the operatorsfingers, (previously lubricated with hogs lard,) till the wholehand can be introduced into the vagina, and slipped up, in aflattish form, between the posterior part of the pelvis andchilds head. This last is then to be raised up as high as pos-sible, to allow room for the fingers to reach the ear and poste-rior part of the neck. When the position of the head is known, the operator must withdraw his hand, and wait to see if thestretching of the parts will renew or increase the labour-pains,and allow more space for the advancement of the head in thepelvis. If this, however, proves of no effect, the fingers an*again to be introduced as before, and one of the blades of theforceps (lubricated with lard) is then to be applied along theinside of the hand or fingers, and left ear of the child, asrepresented in the plate But, if the pelvis is distorted, and TUle 757 projects forward at the superior part of the os sacrum, and theforehead, therefore, cannot be moved a little backwards, inorder to turn the ear from that part of the pelvis which pre-vents the end of the forceps to pass the same; in that case, Isay, the blade must be introduced along the posterior part ofthe ear at the side of the distorted bone. The hand that wasintroduced is then to be withdrawn, and the handle of the in-troduced blade held with it as far back as the perinseum willallow, whilst the fingers of the other hand are introduced to theos uteri, at the pubes or right groin, and the other blade placedexactly opposite to the former. This done, the handles beingtaken hold of and joined together, the head is to be pulledlower and lower every pain, till the vertex, as in this plate, isbrought down to the inferior part of the left ischium, or belowthe same. The wide part of the head being now advanced tothe narrow part of the pel
