Amputations by Long An-terior and Short Posterior Flaps.—It is assumed that the femur is tobe divided about its centre. Thetwo flaps are U-shaped. The an-terior flap is equal in length to onediameter and a half of the limb atthe saw-line. The posterior flap hasthe length of one-half the diameterof the extremity at the same anterior flap is the wider, itsbase slightly exceeding half the cir-cumference of the limb (Fig. 423, a). Operation.—(1) The limb is ro-tated outwards on the right side, and inwards on the left. Theanterior flap is marked out the right thigh the surgeoncommences with the inner limb ofthe flap, cutting downwards. Hethen carries the knife across the front of the extremity, andfinishes with the outer limb of the flap, the leg being now-rotated inwards. In dealing with the left the conditions arereversed, and the cut is first made upon the external incision includes the nteguments only. FIG. 423.—A, Amputation of thethigh by long anterior and shortposterior flaps ; B, Disarticulationat the hip by antero-posteriorflaps. 656 AMPUTATIONS [part vi In marking out the posterior flap the surgeons hand is beneaththe thigh, and the knife, being entered at the farther limb ofthe anterior flap, is drawn across the posterior surface andtowards the operator. This cut also involves the integumentsonly. The leg is again suitably rotated as the knife passes alongits course. (2) The tissues of the anterior flap are now pinched up withthe left hand, and the muscles contained therein are dividedobliquely from without inwards— from the skin to the soft parts are so cut that the flap is thinnest at its extremityand thickest at its base. At the latter site it will include thewhole thickness of the muscular mass in front of the tissues are divided obliquely (the edge of the knife beingturned towards the bone at the base of the fla
