At the midpoint of an imaginary line drawn between thetips of the two pterygoid processes, a trephine of small calibre(%~H inch) is applied and when the button of bone is removedthe hypophysis will be found lying at the bottom of the trephineopening. With a pair of forceps the hypophysis is removed with-out any difficulty as it lies quite loose and unattached in the sella.
