Thick neck in an obese 76 year old male patient with Pickwickian appearance suffering from obstructive sleep apnoea and chronic obstructive pulmonary disease (COPD). Sleep apnoea in which breathing is interrupted during sleep, is common in overweight men. COPD is a combination of chronic bronchitis and emphysema, usually due to heavy smoking. The patient has a nasal cannula which delivers oxygen.
