It is much the same shape as Wells ovariotomy clamp, but is much longer, being nine inches in length including thehandles, thinner, and fitted with movable handles so arrangedthat they cannot slip when the clamp is used. A single ligatureis placed round the stump to prevent it spreading out too much; HYSTERE CTOMY. aa: the clamp is applied in the groove made by this ligature andthe blades are fixed by the screw, after being pressed closelytogether. The tumour is then cut off, the return blood fromthe growth being prevented from entering the abdominal cavityby the assistant. The stump is trimmed, the opened uterinecavity is cut out, and the parts round it are washed with astrong antiseptic solution. The angles of the stump, wheremost of the vessels are usually situated, are ligatured beyondthe clamp, and any large vessels in the capsule must also betied. Sponging is completed, and special care must be given tothe pouch which is formed in front of the pedicle. The stump. Fig. 32.—Koeeerlees Seere-Xceud. must be fixed in the wound in such a way that the peritoneumis brought so closely round it that discharge cannot pass into theabdominal cavity, and it is to assist in doing this that theperitoneum at the lower part of the wound is not open to itsfull extent. This can be done in the following way: one stitchis passed through both sides of the abdominal wall immediatelybelow the stump and so close to it that the peritoneum isbrought firmly against it. The second stitch is put in abovethe stump and also close to it, so as to bring the peritoneumround the upper part and at the same time draw up the peri-toneum of the sides of the wound. The peritoneal cavity isthus closed to any discharge which might gravitate from thecut surface of the stump. When all the stitches have been put 444 FIBROID TUMOURS OF THE UTERUS. in and tied, the dressing is arranged round the stump and un
