Chronic pyelonephritis. Cut specimen of human kidney affected by xanthogranulomatous pyelonephritis. 'Xanthogranulomas' are the yellowish masses of healing tissue produced in response to this chronic bacterial infection. This kidney has been severely damaged, showing suppurating and necrotic areas in the medulla (whitish area, top) and in the pelvis (cavity at bottom of specimen). Chronic pyelonephritis may develop after attacks of the acute form (commonly caused by Escherichia coli). Symptoms of acute pyelonephritis include low back pain, fever and weight loss; in chronic disease the kidneys become small and scarred, and kidney failure may develop.
