The above muscles are solelysphincteric in function. The deep group of musculo-fascial structuresconstitutes the pelvic diaphragm. This is a funnel-shaped structure, oval in shape, with the long axis antero-posterior. It consists of the 564 GYNECOLOGY levator ani and coccygeus muscles of each side, with the visceral layerof the pelvic fascia, which forms their aponeurosis. Anatomists now describe the levator ani as comprising two muscles, which have been named from their attachments, the pubo-coccygeus,and ilio-coccygeus muscles ; the former represents what was formerlydistinguished as the anterior portion, the latter, as the posterior portionof the levator ani. The pubo-coccygeus arises from the back of thepubic symphysis and body, and from the pelvic wall along an oblique line LEVATOR ANI {PUBO-COCCYGEUS ILIO-COCCYGEUS) PUBIC CRisr WHITE LINE. The Pelvic Diaphragm as seen from above (Deaver) which is continued upwards and outwards towards the obturator canal, and also in part from the triangular ligament {see Fig. 296). The musclepasses backwards and inwards and is inserted partly into the lastsacral vertebra, and partly into a tendinous median raphe situatedbehind the rectum, where it unites with its fellow from the oppositeside (Thompson). These muscles partly ensheath the vagina andrectum, and exert to some extent a supplementary sphincteric ilio-coccygeus arises sometimes from the ilio-pectineal line, andsometimes from a fibrous arch passing from the pubes to the ischialspine, the white line of the pelvic fascia (Thompson). It has anaponeurotic insertion into the two last coccygeal vertebrae and intothe ano-coccygeal median raphe. The coccygeus, or ischio-coccygeusmuscle arises from the ischial spine, and is inserted into the l
