The original copy provides for the following entries: 1. The date on which the order is issued. 2. The serial number of the order entered by the secretaryto aid in identifying it. 3. The name and address of the vendor. 4. A blank with no entry requests prepayment of transpor-tation, with Do Not inserted requests that transportationcharges be not prepaid. 5.
