Fig. 4317.—Replacement of the Uterus with Vulsella and the Finger inthe Rectum. The patient is in the knee-chest position. (Hart andBarbour.) doubling up in one direction, while it is perfectly rigid inthe opposite direction. The advantage of this is that itfinds its way into a flexed uterus, and on beingturned over and pushed on it straightens and re-places the uterus; then, by virtue of the tinger-. modifications, notably Albert Smiths (Fie;4320). In those rare cases of recent backwadislocation caused by a fallor some similar accident, it isbest not to apply the pessaryat once, but towait for a timeto see if theuterus will re-main in itsnormal posi-tion after reduction. In suchcases the patient should bedirected to avoid carefullyall lifting or straining move-ments, and to avoid lying onthe back. She should also be di-rected to take the knee-chest positionevery day (vide Fig. 4315). If theuterus remains in good position fortwo weeks it is quite probable thatit will continue so. In this case at-tention should be given to generalhealth, gymnastics, all ordinary cases which havecome on gradually, or have existed fora long time, immediate support will benecessary. If the uterus is very tender,as often happens in these cases, or if thereis doubtful tenderness aboutthe organ, it is often best tohold the uterus in position bypacking thevagina light-ly with plug-gets of cottonsoak
