This 1870s gentleman's portrait shows his large goiter. A goiter is a medical condition caused by an abnormal enlargement (swelling) of the thyroid gland, which is in the base of one's neck, just below the Adam's apple. The most common cause of getting a goiter is the lack of iodine in one's diet. Having a goiter of this size is usually not painful, but the increased size might create pressure in the throat causing a cough or even trouble with swallowing or breathing.
