Clinical gyncology, medical and surgical. pregnancy is more apt to begradual, while after abortion or delivery at full term it takes place sud-denly, in consequence of the rapid lessening in the size of the uterus andthe consequent sinking of the tumor towards the pelvis. The primary result of torsion is interruption of the circulation in thevessels of the pedicle, the consequences of which are more or less serious, according to the suddenness and completeness of the arrest; this dependsupon the length and thickness of the pedicle and the tightness of the tor-sion. As the thicker-walled arteries resist compression longer than theveins, blood is carried to the tumor, but is unable to return, so that extremevenous congestion results. The veins become enormously dilated and mayrupture, thus increasing the tension of the cyst-wall, which may also rupture, fatal hemorrhage occurring. This is, fortunately, rare, the more commonresult being moderate hemorrhage or extravasation. NEOPLASMS OF THE OVARIES, TUBES, AND BROAD LIGAMENTS. 651. mm Adhesions on tlie surface of an ova-rian cyst which had been separated fromits pedicle by torsion. (Doran.) The appearance of a rotated cyst at the operating-table is of the usual white, glistening hue, it presents a deep-red, some-times almost black, color, the veins nearthe pedicle being prominent. Section ofthe cyst-wall shows that its color is duenot to gangrene, but to the uterine conges-tion and extravasation. The fluid maypresent the ordinary chocolate color, or maybe black and tarry from the presence ofcoagulum, as in a case in which the writerhad an opportunity to make a post-mortemexamination. While necrosis may resultactual moist gangrene of the cyst must berare it there has been no infection fromwithout. What does occur in these acutecases is the rapid development of perito-nitis, which is naturally of a virulent twisting of a coil of intes-tine, leading to complete obstruction, hasbeen
