The increasein volume is not a true hyperplasia, forthe glandular functions, as well as the mus-cular power of the organ,decreases with thehypertrophy. In some portions of the massthe muscular tissue is increased, but thebulk of the enlargement isdue to the pres-ence of newly-formed connective tissue. The induration is in proportion tothe excess of the new tissue over the nor-mal muscular and glandular elements. In general hypertrophy, while the increase in size is in all directions,it is more marked in the posterior portion, where it encroaches on theneck of the bladder. Not infrequently one lateral lobe is greatly enlarged, or the hyper-trophy may be central, resulting in the development of a middle or thirdlobe, which, by progressive enlargement, not only changes the axis of thenormal urethra, but occludes in a variable degree the outlet of the bladder. Probably venereal excess is the most generally accepted cause of pros-tatic hypertrophy. Symptoms.—The first symptom which attracts the attention of thepatient is the obstructed flow of urine. As the obstruction becomesgreater, retention of more or less urine occurs, which in time sets up achronic cystitis, and then urination is more frequent and becomes a painfulact.
