We haveoften used for the purpose theEcraseur forceps here shown(Fig. 19) which are of our owndevising, though by an accidentthey are credited in the instru-mental catalogues to ProfessorByford. The error was not dueto him, as he never laid anyclaim to the invention. The instrument is easilyFig. 18.—Smiths Cauteky Clamp, gleaned, simjjle in structure, and much more easily applied than the complicated chain6crasem-, and does not break like the wire 6craseur. Theeffect on the pile is exactly the same as that of the others, neither better nor worse. Smith, of London, has devised. HEMORRHOIDS OR VILKS. 39 / a small pile ecraseiir (^Fig. 20 j, using a wire cable a chain. The pile being seized with viilsel-Inni forceps, the instrument is ap]iliedat the same point where a ligaturewould be, generally half way betweenthe base and the summit, and slowlytightened until the tissues are patient must be kept some days inbed, the stumps are treated antisepti-cally, and the general management isthe same as after ligature. As in allother methods of removing piles, it isnecessary to be careful and leave suffi-cient mucous membrane to make a dis-tensible vergfe of the anus, otherwise astrictiue will follow. The ecraseur cures piles effectu-ally, but it is occasionally followed bydangerous lijBmorrhage, so that its popularity in this country has decidedlywaned. In our own practice we havediscarded its use. Crushing the Pile.—Anothermethod consists in crush
