Where the difiiculty in the permanentremoval of the tube is due to the presence of granulations in the trachea orlarynx, after their removal by some of the methods before mentioned the phy-sician is usually able to dispense with the tube. Where stenosis of the tracheaor larynx exists and prevents the permanent removal of the tube, the partsmay be gradually dilated by the use of bougies, or, better, by the introductionof an intubation-tube after the removal of the tracheotomy-tube: the wound inthe neck can then be plugged with a nipple attached to a shield (Fig. 6), or with Fig. 6. ^- ^f\.
