His mother Jill is holding a mirror so that James may see his own face. Infants confined for long periods in such an unnatural environment experience problems with their personal development. The mirror is aimed at making him more aware of his own body. An iron lung is a type of negative pressure ventilator, a device that exerts external pressure to cause the lungs to expand and contract.
