Modern surgery, general and operative . (d) isfitted upon a long metallic stem, and carried through the small cylinder to theend of the instrument, as shown in the illustration. The proctoscope is intro-duced through the anus with the obturator (o) in position. As soon as theinternal sphincter is passed this obturator is withdrawn and the bayonet-fitting plug (b), which contains either a plain glass window or a lens focused tothe length of the instrument to be used, is inserted in the proximal end of theinstrument. This plug is ground to fit air-tight and thus closes the instru-ment perfectly.

The plug being inserted in the tube, a very slight pressureupon the hand-bulb will cause inflation of the rectal ampulla to such an extentthat the whole rectum can be observed and the instrument can be carried upto the promontory of the sacrum without coming in contact with the rectalAvail. Further dilatation will show the direction of the canal leading intothe sigmoid, and, by a little care in manipulating the instrument and keeping.
