Small lymphocytic lymphoma, light micrograph. B-cell chronic lymphocytic leukemia and small lymphocytic lymphoma are the most common mature B-cell neoplasms and are considered to be different manifestations of the same disease. Chronic lymphocytic leukemia affects middle-aged and elderly patients. The mature-appearing neoplastic B-cells are predominantly in blood and bone marrow. In small lymphocytic lymphoma, the neoplastic cells are mainly extramedullary with very few leukemic cells in blood. This image shows small lymphocytic lymphoma involving the skin. Nests and cords of neoplastic B-lymphocytes (dark blue areas) are separated by collagen bundles (pink areas) in the dermis.
