The international encyclopaedia of surgery; a systematic treatise on the theory and practice of surgery . septum is now to be sought for, andmay be recognized (a) as the first intermuscularspace from the tibia; (6) on pressure from withinoutward, by the resistance of the other muscles; (e)at the lower part of the incision, by the white line ofthe muscular interspace being more marked. Thefoot being flexed, separate the muscles with the in-dex finger; and, the margins of the wound beingdrawn apart by retractors, expose the artery with itsvenffi comites and the anterior tibial nerve, thelatter of which is outside. Pass the needle fromwithout inward. Operation at the 3IidcUe Third.—In this part of itscourse the artery is covered by the skin, the super-ficial, and the deep fascia. On the inner side ithas the tibialis anticus muscle, and, on the outer, the extensor longus digitorum and the extensorproprius pollicis (Fig. 508). Having placed thelimb in the same position as directed above, make an incision three and ahalf inches in length, along the course of the artery, and through the integu-. Ligation of the auterior tibial ar-tery at the middle third of its course.(Sedillot.) 324 INJURIES OF BLOODVESSELS. meiits. The septum in the deep fascia is to be recognized by its white it longitudinally, and likewise by a cruciform incision. Flex the footto relax the muscles, when, the sides of the wound being separated by draw-ing the tibialis anticus inward, and the extensor longus digitorum and ex-tensor proprius pollicis outward, the anterior tibial nerve is found lying moresuperticial than the artery and its accompanying veins. Pass the needle fromwithin outward. Operation at the Loiuer Third.—Here the artery is covered by the integu-ment and the fasciae, and is crossed by the tendon of the extensor propriuspollicis. Low down, it lies between the tendon of this muscle and the ten-dons of the extensor longus digitorum. It is accompanied by two veins andthe ante
