It is important that the padding be 282 FRACTURES OF THE FEMUR evenly and intelligently arranged. It will be necessary to placea wide pad between the upper end of the long outside splint, toprevent it from pressing upon the ribs and side of the chest andcausing great discomfort. These splints are held in positionabout the leg, while gentle traction is being made upon thelimb by straps or pieces of bandage placed above the ankle, below the knee, above the knee, at the middle of the thigh, and at the level of the perineum. The patient should then be carefullyplaced upon a stretcher (a Bradford frame is an ideal form ofstretcher) improvised for the occasion. With this apparatussnugly applied, the patient may be securely and comfortablytransported. The objects of treatment are to reduce the fracture, to main-tain the reduction immobilized until union is firm, and to restore THE SHAFT <)!• THE FEMUR 283 the leg to its normal usefulness. In the treatment of two of thethree varieties of fracture of the femur permanent traction uponthe lower fragment and permanent countertraction upon theupper fragment are necessary.
