If for some reason the tumor causes a rotation in suchan elongated cervix, hydrometra or hsematometra is apt to occur; or thepedicle may be twisted the same as in an ovarian tumor and gangrene mayset in, or it may be entirely separated from the uterus and may remainperfectly innocuous in the abdominal cavity. Localized peritonitis is notinfrequently found, and the neoplasm forms attachments to the surroundingparts, as the abdominal walls, the intestines, or the bladder; the nutrimentof the growth may be principally derived from vessels in the adhesions ofthe surroundings. If the pedunculated growth gravitates into the posteriorcul-de-sac, symptoms referable to the bladder and rectum will occur. With the tumor on the anterior surface ofthe womb, that organ is soon retroposed, and we obtain the symptomsof retroflexion in addition to those of anteflexion. Intestinal obstructionmay be caused by rotation of a pedunculated tumor adherent to a portionof intestine. The tumors of this class also develop into the folds of thebroad ligament, but, fortunately, this is infrequent. The longer and thinnerthe pedicle the more independent are the movements of the tumor in theabdominal cavity and the greater are the chances that, as the result of localirritation, more or less ascitic fluid will accumulate in the peritoneal cavity. 556 BENIGN NEOPLASMS OF THE UTERUS.
