The aperture is fashioned to fit the part to be pro-tected and the long axis of the shield is allowed to restacross and upon the adjacent toes. The writer lias observed many cases where theproximal phalanges of the intermediate toes were in a stateof constant extension due to the contraction of the extensortendons, and leaving a decided hollow in the dorsum of thefoot directly over their metatarsophalangeal joints. In acondition of this nature a thick shield of this pattern is par-ticularly practical and may be used not alone to protect atender part, but also to fill up this hollow and allow theshoe to fit more firmly. Where helomata are present on the dorsum of all ofthe phalangeal joints, this variety of shield is, of course,contra-indicated, as undue pressure would be brought tobear upon the already troublesome excrescences and a greatamount of trouble would in this way be invited. In someinstances a full oval shield is not used, but a dorso-digitalhalf-moon, as shown in Fig. 1-Gr, is substituted. No definiteruling can be made as to which form of shield should beused, as each
