While there is considerable atrophy of 128 GENERAL DIAGNOSIS. the muscles there is also change in the ends of the bones and ends of the bones are enlarged and the cartilages undergo joints of the phalanges may be swollen and the tissues infiltratedprior to the destruction of the cartilage. This may have been presentfor a long time, increasing in amount at different periods with pain andtenderness. The joints gradually become more immobile, the infiltrationdisappears, and the enlarged ends of the bones become more or less ankylosis develops, and on motion crepitus and grating isfelt on account of the eroded cartilage. Osteophytes may form in the Fig. Rheumatoid arthritis. The phalangeal joints are swollen; many are ankylosed. The wrist isstiff. The muscles are atrophied ; the forearm muscles much wasted. tendons, so that the joint becomes more completely locked. Atrophyof muscles supervenes on account of the disease of the joint. Some-times the wasting is very extreme and gives the hand the appearancethat is seen in pseudo-muscular atrophy. The general symptomsthat attend each affection serve to distinguish them. Rheumatoidarthritis is easy of recognition when the other joints are involved inthe process. Contractions of the hand may often be observed from other causesthan the ones just mentioned. Temporary contractures occur in tetany,in temporary hemiplegia or monoplegia, and in paralysis of the exten-sors.
