I have seen patients whowere subject to these attacks everyfew weeks followed by immediaterelief after the reduction of thehernia, which they had learnedto do themselves. Sooner or latersuch hernise are pretty sure tobecome strangulated, and the stran-gulation may lead to a fatal re-sult before an operation can beperformed. Incarceration or persistence ofthe contents in the hernial sac, dullpain in the region of the hernia in-creasing with exertion, or gradualenlargement of the sac, are signsdemanding opei-ation, unless such contra-indications as grave cardiac lesion, advanced nephritis, asthma, or great obesity exist.
