The size of the perforation varies from that of the prick of a needleto the complete destruction of the membrane. But the extent of theloss of substance depends neither on the duration nor on the in-tensity of the purulent process. The greatest loss of substanceoccurs with scarlatino-diphtheritic middle-ear suppuration and intubercular phthisis of the mucous membrane of the middle
