Plastic surgery; its principles and practice . en with a chiselthe superficial portion of the outer table of the bone is raised with theflap, the bone is broken across at the base and the flap is turned flap consists of a narrow strip of skin with a thin layer of the frontalbone, and should be long enough to reach the cartilaginous edge of the 464 PLASTIC SURGERY gap in the nose when the tip is in its normal position. The epithehalsurface of the flap above the opening into the nose is denuded, and theend is then stitched to the cartilaginous portion, so that its skin surfacecovers the opening into the nose. The lateral skin flaps are loosenedand closed over the raw surface of the bone flap in the midline and thewound on the forehead is sutured. Three weeks later the pedicle ofthe reflected flap is severed, and the parts adjusted. The authors admit that there is usually sinking of the bridge whenthe operation is performed as above described, so that secondary-implantation of bone or cartilage becomes necessary (Icig. 501)..
