There is, however, a form of gangrene inthis connection which occurs occasionally and which exhibits marked charac-teristics. This form is called spontaneous gangrene. It appears suddenlyand with little or no warning. The scrotum quickly swells, often to a greatsize. At first it is red. This color, however, rapidly becomes dusky, andsoon the parts markedly involved become dark gray or black. After a dayor so the line of demarcation forms, and then gradually the lifeless tissues 1. Sometimes this destructive process is so extensive as to involve thepenis as well as the scrotum. The exact cause for this form of gangrene isnot known, though it is probably of bacterial origin. It occurs mostly inthose addicted to alcoholic excesses, making its appearance directly after orduring a prolonged debauch. Some of these cases are complicated by anemphysema. Surgical treatment in these conditions should always be mostconservative. General supporting measures should be employed, togetherwith careful attention to nutrition. They should not, however, be cutaway along the line of demarcation, lest
