. A treatise on the diseases of the eye. sudden gush of the contents, when the[Fig. 96.] anterior part is removed. 2. That the points of emergence indi-cate the lines of incision. 3. That the presence of the needlesprevents, or rather restrains, to some extent, the escape of thelens and vitreous humor, after the anterior part of the staphy-loma has been removed. The next stage of the proceeding is toremove the anterior part of the staphyloma. This requires somejudgment and modification in size and form, in accordance withthe extent of the enlargement, so as to leave a convenient usual plan is to make an opening in the sclerotic, about twolines in extent, just anterior to the tendinous insertion of theexternal rectus, made with a Beers knife [Fig. 96]. Into thisopening I insert a pair of small probe-pointed scissors, and cutout an elliptical piece, just within the points where the needleshave entered and emerged. The needles, armed with fine blacksilk, are then drawn through each in its turn, and the sutures Fig. After Lawson. are carefully tied, so as to approximate as closely as possible thedivided edges of the sclerotic and conjunctiva (Fig. 97). Theoperation is now finished; the speculum may be removed so asto allow the lids to close, and wet lint may be applied to keepthe parts cool. In a large majority of cases, union of the dividededges takes place by the first intention. . , I gener-ally leave the sutures in for some weeks. Sometimes they comeaway spontaneously, and when this is not the case, they mayreadily be removed after all irritation has passed away, and afterfirm union has taken place. If the case be examined three orfour months after the operation, a movable globe is seen with aflattened anterior surface, traversed by a white line of cicatrix, and havingrather a prominent external angle. Upon this an artificial eye can bereadily adapted, which moves to a greater extent than I have observedprevious to adoption of my present method. 260 DI
