In order to prevent the growing together of the two ends, oneshould try, if the field of operation permits, to remove a piece of the processby a second incision. In several cases operated on by the author, the improve-ment in hearing was remarkable and lasting ; often, however, it was transientor nil. In two cases in which incision of the cicatricial tissue around theadherent handle had no effect and the incudo-stapedial articulation was in-visible, a marked improvement in the hearing resulted after cutting through thehandle of the malleus above the short process with the instrument described,followed by its extraction with Sextons forceps.
