At the same moment lateral pressure should hemade upon the projecting bone in the direction of the articulation. Itis of some use, also, occasionally to flex and extend the limb moderately, and to give to the foot a gentle rocking motion. If more force is needed,it may be applied by placing the limb over a firm double-inclined frac-ture-splint, and making the extension by the aid of a screw attached tothe foot-board, as I have suggested in certain cases of dislocation at theknee. Or we may employ the pulleys after the manner represented inthe accompanying drawing, Fig. 492. C. S., aged about 30 years, while carrying a weight upon his shoulders, onthe 6th of May, 1854, slipped upon the sidewalk, and fell, dislocating the left Fig. tibia inward, and fracturing the fibula four inches from its lower end. I was inattendance soon after the accident occurred, and found the tibia projectinginward, with the other symptoms usually accompanying a simple rotation of theastragalus upon its axis. Seizing the foot with my hands and flexing the leg, while an assistant held the thigh and made counter-extension, I had scarcelybegun to pull upon the foot before the reduction was effected. Dupuytrenssplint was at once applied, and the subsequent inflammation was so trivial asscarcely to deserve notice. In six weeks the limb was sound, and free from allankylosis. 774 DISLOCATIONS OF THE LOWEE END OF THE TIBIA. In my report on dislocations, made to the New York State Medical Societyfor the year 1855,1 have mentioned twelve similar examples, in addition to someexamples of compound dislocations, all of which were easily reduced, but theresults were not always so favorable.
