In a proportion of the cases of gastric disorder it isfound that the opaque food is kept in the upper part ofthe stomach for some time, and part of it passes througha narrow channel near the lesser curvature, so that thelower sac gradually fills. At a certain stage we see themeal more or less equally divided between the upper andlower sacs with a narrow channel between—the hourglass stomach. (See Fig. 65 and Plate XIII.) This condition is practically always due to the presenceof gastric ulcer; in the early stage the constriction is 174 TEXT-BOOK OF spasmodic arising from the irritable state of the ulcer;at a later stage the constriction may be organic, the resultof cicatricial contraction. See Fig. 66. As regards the constriction and also the symp-toms, the spasmodic element isjust as important as the food is observed to be heldup in the upper part of thestomach longer than usual, thefront of the abdomen should berubbed more or less vigorously, ifnecessary, and it frequentlyhappens that the spasm is relievedand the food passes easily to the lower part.
