Diffuse large B-cell lymphoma, light micrograph. Diffuse large B-cell lymphoma is the most common type of lymphoma and accounts for about 35% of all cases of non-Hodgkin lymphoma. It is an aggressive malignancy and presents with rapidly enlarging lymph nodes or tumor masses at extranodal sites. There is diffuse proliferation of large to medium-sized lymphoid cells which can be classified into centroblasts and immunoblasts. Some cases have markedly pleomorphic anaplastic cells. This is a photomicrograph of diffuse large B-cell lymphoma of stomach in a gastric biopsy from a patient who presented with gastritis. The tumor cells show strong immunoreactivity for BCL6. About one-third of diffuse large B-cell lymphomas have rearrangements of the BCL6 gene located on chromosome 3q27. An even higher fraction of cases have activating point mutations in the BCL6 promoter resulting in accumulation of BCL6 protein.
