Failure of Driving Axle Caused by Fracture Starting at Bottom ofHoles Drilled for Plugs Used In Attempt to Tighten Wheel Fit the year ended June 30, 1919, covering the entire locomotiveand tender and all of their parts and appurtenances, shows anincrease of 49 per cent in the number of accidents, an increaseof 16 per cent in the number killed, and an increase of 42per cent in the number injured. This increase is due almostwholly to disregard for the requirements of the law and rulesas well as to safety of construction and operation. This isespecially true with what are sometimes considered unimpor-tant parts; for instance, 26 per cent of the increase in acci-dents and injuries was due to failure of grate shakers; 10per cent was due to failure of reversing gear; and 10 per centto failure of squirt hose.
