Only when the conditions are very favourable, as wherethere is a wide external meatus and a great distance betweenthe promontory and the remnant of the membrane, are we in aposition to inspect different parts of the upper and posteriorspaces of the tympanic cavity by intratympanal otoscopy; thisprocedure is carried out with a small tympanic mirror, as re-commended by Blake and Bing, under strong illumination, andwith the aid of a magnifying lens. By gently turning the smallmirror on its long axis we can examine successively the superior,posterior, and anterior portions of the tympanic cavity, andsometimes the deep-seated floor (cellar), and are thus able torecognise many important diagnostic changes.
