C 1930's - An old school English regional education certificate ( County School, Whitby, Yorkshire) that had to be completed by each pupil before returning to school after the holidays to ensure no contagious or infectious diseases were brought into school. Parents were also expected to report knowledge of any such diseases or illnesses that they had come across in their home, church or neighbourhood such as polio,tuberculosis and influenza
