Insulate the patient negatively, ground the positive poleand the brass ball electrode to the gas fixture. Apply mildsparks to the affected joint, or stronger ones if they are welltolerated. Request the patient to bend and pose the joint tobring out the local pains during treatment, and in each positionspark the tissues until relief ensues. Repeat every second dayuntil improved and then three times a week until benefitceases. If the part is still sensitive and sore to the touch sparks willnot be tolerated, and the Leyden-jar sedative-tonic applicationmay be made far more agreeable. Apply two sponge-coveredhand electrodes to the opposite side of the joint, connect thesmallest jars in circuit with a short spark-gap, rapid action ofthe plates and a mild Fig. 366. Fine felt or sponge covered electrodes—assorted sizes with soft rubber insulating backs. Pass one electrode very slowly over the area affected whilethe other is held stationary. The question of polarity is notimportant, and if the negative electrode is somewhat larger thanthe positive the sensation of the two will be about the same. Continue the application until all stiffness and pain are re- 8o8 TREATMENT OF ARTICULAR RHEUMATISM. Heved. If any particularly sensitive spot is found, hold thepositive electrode upon it, with a regulation of the dose at thepoint of comfortable tolerance, until sedation is complete. The constant galvanic current may also be employed withthe electrodes on opposite sides of the joint. Regulate thecurrent strength to comfortable tolerance (from lo to 20 mil.).
