PATHOLOGY OF THE DECIDUA AXD OVUM. 227 Endometritis.—One of the most generally observed probably dependson endometritis antecedent to conception. When the impregnated ovulereached the uterus it engrafted itself on the inflamed mucous membrane, which was in an unfit condition for its reception and growth. A notuncommon result, under such circumstances, is the laceration of some ofthe decidual vessels, extravasation of the blood between the decidua andthe uterine walls, and consequent abortion at an early stage of preg-nancy. As this morbid state of the uterine mucous membrane is likelyto continue after abortion is completed, the same history repeats itselfon each impregnation, and thus we may have constant early miscarriagesproduced. It does not necessarily follow, however, that the pregnancyis immediately terminated when this state of things is present. Sonie~ Fig. Hypertrophied Decidua laid open, with the Ovum attached to its Fundal Portion.(After times a condition of hyperplasia of the decidua is produced, the mem-brane becomes much thickened and hypertrophied in consequence ofproliferation of its interstitial connective tissue, and the decidual cellsare greatly increased in size (Fig. 87). In other instances the internalsurface of the decidua become- studded with rough polypoid growths,1depending on proliferation of its interstitial tissue. Duncan has foundthat the hypertrophied decidua is always in a state of fatty degeneration, more advanced in some places than in The result of these alter-1 Virchows L861, 1st ed. - Researches in Obsteb , p, 228 PREGNANCY. ations is frequently to produce dwindling or death of the ovum, which, however, retains its connection with the decidua, until, after a lapse oftime, the decidua is expelled in the form of a thick trian
