The aim of the operator is to pass the hook around theneck of the foetus, and this is accomplished as follows: Insertingtwo fingers of the right or the left hand (according as the fcetalhead occupies the left or the right half of the pelvis) into thevagina, the hook is passed flat along these lingers until the neckof the foetus is reached. The point of the hook is then guidedaround the neck by these fingers from above downward, inorder to lessen the risk of injuring the bladder. (Plate XV.) X K Eh< t—i. CD EMBRYOTOMY. 159 Firm traction is then made on the hook in order to assure athorough hold on the neck, the fingers remaining in place soas to certify that the point of the hook is not injuring thematernal parts. The hook is rotated, traction being maintaineduntil the neck is felt to yield through the breaking of thespinal column. As a rule, the soft parts also are thus severed,and the hook is removed along the fingers. If the hook hasfailed to sever completely the muscular attachments, the scissors,guided along the fingers, must be utilized.
