The apparatus ought to be first adapted to thesound limb in casesof simple fracture of the leg, andwell padded ; a small concave pad, too, should beplaced on the inside of the heel of the shoe, andanother pad upon the sole. Two side splints arerequired, the outer one extending from the foot-board to the upper part of the outer condyle ofthe femur, and the inner one from the foot-boardto the inner condyle; — also a split deal shin-splint ; and in cases of oblique fracture, a thin padto be applied upon the instep, covered with a pieceof paste-board, a little wetted, which, when dry, serves to equalize the pressure and keep the in-step easy. Application. — In the first, or Inflammatory MINOR SURGERY. 269 stage. — The shoe, a, containing the heel andsole-pads, should be carefully placed upon thefoot; the instep pad should then be placed uponthe instep, and the shoe closed over it, and closely Fig. confined to the foot by means of the buckles andstraps attached to it for that purpose. An assist-ant should then place one hand under the knee, and, taking the foot in the other, raise the fracturedlimb, bringing it round so as to let it rest uponthe heel. When the limb is raised, the surgeonplaces the apparatus under it, and brings the ankleof the same opposite the bend of the knee, direct-ing the assistant to lower the limb upon it. The surgeon now fixes the shoe, a, to the foot-board, b, by means of the straps attached to thesole. By the aid of this shoe, he is enabled toraise or lower the foot according to the length ofthe heel or thickness of the calf, so as to bring thelower portion of the fractured bones into a properline with the upper, as far as respects any angularprojection backward or forward. A paddedsplint should be placed upon the front of the thigh, and the whole of the thigh-part of the apparatus23* 270
