When,in these cases, the bony wall in the direction of the antrum was ofnormal consistency, the author has never tried to make a com-munication between the abscess cavity and the mastoid antrum. If one is convinced that the abscess cavity is thoroughlyscraped out after having used the smallest sharp spoon, andsatisfied, with the aid of the probe and sharp spoon, that thecavity in a direction inwards and forwards towards the antrum is. Fig. 258. bounded by resistant, healthy, bone tissue, the cavity is tam-poned with iodoform gauze without previous irrigation. In orderto make the wound somewhat smaller, several sutures are placedin its superior and inferior extremities, the number of whichdepends on the loss of substance in the bone. In superficialabscesses with slight loss of bone substance, the wpund maybe sutured immediately after the operation after having lightlydusted the parts with iodoform (Gruber). When there is a largeabscess cavity it is not advisable to allow the wound to close upby primary union, because, when extensive softening exists, piecesof diseased bone may occasionally be retained, which would makeit necessary to repeatedly scrape out the cavity during the after-treatment.
