Diseases of the nose and throat; a text-book for students and practitioners . ough much more force is required topush such an edge through the tonsil. In order toovercome this clanger and to facilitate the application of theloop and subsequent manipulations, Dr. Knight {Trans. Asson, 1889) devised a galvano-cautery snare modeledafter Mackenzies tonsillotome, but the blade of the latter isreplaced by a straight, transverse piece of platinum wire. Theoperation is performed as easily and as painlessly as with thecutting tonsillotome. In this procedure two points should beremembered: (a) if the wire be hot enough to ablate the tonsilquickly there is danger of haemorrhage, which will not occur ifthe division be rather slow; and (b) the destruction of tissue. In using Jarvis snare or the wire-loop ecraseur, the same process is carried out as that recom-mended in the removal of nasal tumors. It requires from oneto two hours, depending upon the size and structure of thetonsil. The bistoury and forceps are now rarely employed. In all of these operations it is important to avoid woundingthe faucial pillars, lest loss of control of the muscles result orannoying haemorrhage follow. The uvula is occasionally burnedwith the galvano-cautery, or amputated in a hasty operationwith tonsillotome or knife and scissors. This may interferewitli the singing voice.
