PRESBYOPIA SURGERY, ILLUSTRATION


Treatment of presbyopia by scleral expansion (refractive surgery) : scleral incisions. Technics of scleral expansion relies on the controversial theory that the loss of accomodation of presbyopia is not linked to the loss elasticity of the eye lens, but to the lack of space. In fact, the diameter of the eye lens increases naturally with years, so that they would not be enough space to curve and accommodate (theory of Schachar, 1995). Enlarge the diameter of the sclera would provide the eye lens the space it is lacking. Two technics of scleral expansion are used : scleral incisions (Laser reversal of presbyopia and Anterior ciliary sclerotomy) and strips of scleral expansion. Scleral incisions : eight incisions in triangle are made in the sclera. This increases the capacity of expansion of the sclera and restore the accomodation. These incisions are undertaken either mecanically (Anterior ciliary sclerotomy), either by infrared laser (Laser reversal of presbyopia, YAG laser).
