There was alarge patch of craniotal)es in the occipital region. In the midoccipital region,about at the torcular, there was a deep follicle from which several hairs grewlike a pilonidal sinus. There was a huge median vein, 1 cm. in breadth, com-ing from the torcular. Examination of the fundi showed bilateral early chokeddisks of low elevation. Operation.—On Feb. 3, 1920, Dr. Gushing made a suboccipital explorationfor possible tumor of the cerebellum producing Fig. 9 (Case 3).—Interior of lirain, showing extreme degree of internalhydrocephalus. The suboccipital region was exposed by reflecting the skin and muscle flapsafter the usual crossbow incision. Just above the operative area there was atangle of dilated veins, radiating like a caput medusae from the torcular. The occipital bone was exposed without great difficulty. It was not espe-cially thin, nor was there any protruding of the suboccipital region. The dura,on exposure, was found to be tense, and a lumbar puncture needle was inserted HORRAX—CHOLESTEATOMAS 281
