BENIGN EPITHELIAL GROWTHS OF THE UTERUS 499 Diagnosis. Papuliferous adenoma of the uterus in elderly subjectshas to be differentiated from adeno-carcinoma [see p. 533). The symp-toms and clinical features are similar in each case, therefore the natureof the new growth can only be determined by histological investigationof the scrapings removed by curettage. In some cases this may proveeasy, and no doubt will arise as to the simple nature of the case, the. Fig. 250, Papilliferotjs Adenoma of the Uterus (Malcolm). The growthdid not invade the muscular walls of the uterus. The uterus was also the seatof a medium-sized fibroid. Some of the tubules show epithelial proliferation. tubules being lined by a single layer of columnar epithelium, andbut little or no invasion of the uterine muscle is present. On theother hand there are cases, that reported by Bland Sutton {seeFig. 249), in which considerable difficulty will be found in decidingwhether the amount of penetration of the uterine wall by the gland-tubules constitutes malignancy or not. Curettings obtained from repeated recurrences in some cases haveshown a benign structure, but upon removal of the uterus its wallshave been found to be invaded by a growth indistinguishable froma tubular carcinoma (Targett). 500 GYNECOLOGY Prognosis. From the above facts it will be seen that a villouspapilloma of the uterus, although benign, possesses the power ofinfiltration and invasion of the musc
