From much practical experience with this card system of recordsI can confidently recommend it as the best method of securing andcompiling medical and surgical statistics, and of preserving in convenientform the original data upon which such statistics mast be based, andfrom which new combinations may hereafter be desirable. Herewithare presented the form of card used in the statistical division ofthe Surgeon Generals Office at Washington, and also the form uponwhich cases of injury or surgical operation are reported, together withforms of cards used in the Vital Statistics Division of the UnitedStates Census. The great objection to relying exclusively on forms of reportscompiled by individual medical officers on which the detailed data foreach individual are not given, is that they do not
